Filing a Claim or Appeal for Leumit’s Long-Term Care Insurance

Filing a Claim

To file a claim, you must complete the claim form and attach the relevant documents as detailed.

Claim Process

After your initial contact with the Leumit helpline at Menora Mivtachim, your case manager (a trained social worker) will reach out to explain and guide you through the claims process and help you maximize your rights with various entities.

Once the claim documents are received, the insurance company will verify coverage according to the policy terms.
The company may request a functional assessment performed by a doctor, nurse, or specialist. This evaluation will be pre-scheduled and conducted either at the insured's residence or the care facility where they reside.

If further clarification is needed, the insurance company may contact you to provide additional documents. In some cases, the insurer may directly request the insured's medical file from healthcare institutions.

After receiving the evaluation results and all necessary documents, you will receive a written response regarding your claim for insurance benefits, in accordance with the policy terms.

Please note: Submission of the claim form to the insurer does not constitute agreement, obligation, or approval of the claim by the insurer. You can track your claim’s progress in the personal area of Menora Mivtachim’s website.

If the claim relates to an event that occurred before April 1, 2019, it will be transferred for processing and resolution by Clal Insurance Ltd.
Claims already under review by Clal Insurance will continue to be processed by Clal until a decision is made.
Insured individuals whose claims are approved and receiving benefits from Clal will continue to receive those benefits as long as they remain eligible.

Filing an Appeal

If your claim is denied or if you wish to appeal the determination of your eligibility date for benefits, you must file an appeal within 60 days from the date you receive the denial letter.

Additionally, to fully exercise your rights, you may challenge the insurer’s decision by contacting the Public Complaints Officer at Menora Mivtachim via:

You may also appeal to the Capital Market, Insurance, and Savings Authority at the Ministry of Finance or pursue legal action in court.
